Only thing that's got me stumped is the coolant temp sensor, where is it?

Coolant temp sensor is in the head this gives the ecu the signal. Check icv if you have it in. What is the maf adjusted to? Sounds like it is dying when it comes off choke

    Side or front?

I've got a 2pin (like an injector) connector on the front (flywheel end) but it looks more like a knock sensor then a single pin one on the end of the head above the thermostat housing (which looks more like a temp sensor) but my loom doesn't seem to have a connector to fit it?
